Our client Is a U.S.-based digital health company on a mission to deliver exceptional care experiences for patients. Through a combination of technology and clinical expertise, Their platform serves healthcare organizations across the U.S. and reflects a commitment to innovation, patient-centered care, and operational excellence.
About the Role
We're looking for a backend-first full stack engineer with real end-to-end ownership. You'll primarily design and ship FastAPI services and REST APIs; contribute to member-facing surfaces (web, iOS, Android); and work on APIs for AI consumption and/or EMR integrations.
You'll also have lighter exposure to E2E automation and AWS infrastructure. You'll likely focus on one or more of our core backend domains: Clinical Systems & EMR, Identity & Access, Financial Operations or Patient Interaction, alongside broader API and platform work. All of this sits in a regulated healthcare environment where correctness, security, and user experience matter.
Key Responsibilities
Design, build, and maintain scalable FastAPI services and RESTful APIs across auth,
scheduling, payments, insurance, messaging, and clinical workflows.
Build APIs for AI consumption, well-documented, secure endpoints that AI agents and internal tools can call reliably in production.
Contribute to member-facing web (TypeScript, React, Next.js) and support iOS and Android teams on API contracts and integration debugging.
Architect PostgreSQL schemas (RDS/Aurora), write Alembic migrations, and optimize queries; utilize Redis for performance optimization.
Implement async workflows with Celery, RabbitMQ, and Redis for webhooks, notifications, and background processing.
Monitor and debug production systems using Datadog and Sentry.
Write pytest and frontend tests; update Playwright E2E when product flows change.
Contribute to GitHub Actions CI/CD for automated testing and safe deployment.
Requirements
Experience: 5+ years professional development; 2+ years production Python backend (FastAPI or Django/Flask).
API Design: RESTful APIs powering D2C web and mobile applications.
Deep Domain Expertise: At least one of: Payment systems (Stripe), Auth/Registration (OAuth/JWT), or Healthcare integrations (EMR/EHR, FHIR).
Databases: PostgreSQL (RDS/Aurora), schema design, and SQLAlchemy/Alembic.
Infrastructure: Message brokers (Redis, RabbitMQ) and async tasks (Celery).
Observability: Proficiency in Datadog and Sentry for APM and production debugging.
Frontend: TypeScript, React, Next.js, ability to ship production web UI.
Security: Fundamentals in secrets management and secure auth patterns.
Nice To Haves
Regulated Data: Experience with HIPAA, PHI, or FHIR.
Advanced Sourcing: Experience with Terraform or AWS CDK is highly valued for infrastructure management.
AI/LLM: Designing APIs for AI agent consumption and reliability at scale.
DevOps/Cloud: AWS ECS and core services (VPC, S3, IAM); Kubernetes or Docker expertise.
Testing: Playwright or Cypress E2E ownership.
Data Pipelines: Kafka or streaming systems; OpenSearch/Elasticsearch integrations.
